An appellate lawyer working in the federal courts who claims to be “unfamiliar” with the far right is either irretrievably stupid or pretty ok with them. Ain’t no third choice.

I think his point is that Alito is disturbingly well-versed in a specific flavor of terminally online far-right extremism that, by design, evades normie discourse (e.g. the many variants of boogaloo), not that he’s surprised the far right exists and holds enormous power in government.
