Get a will, get an advanced directive, and talk to your families (seriously!!! don't blow it off with jokes!) about what you want.

I'm so sorry for your burden. My mom recently had her husband pass (she remarried after I moved out), and she was talking about how much of a nightmare it was to handle his affairs and sorted hers out for me and my sister for when the time comes shortly after dealing with it.
Been going through this a lot this year, and even after a life-threatening incident the person in question still won't take care of themselves or get these things in order. It's exhausting. Hang in there :(
