In some good news, most (some say ~all) of the increase in the 2000s is due to the staggered rollout of a new way of counting maternal deaths: pubmed.ncbi.nlm.nih.gov/32510309/

Addendum! US increase largely attributable to introduction of a death certificate checkbox (was decedent pregnant/recently pregnant). When coroners checks the box, it's maternal death.
