My concern is that a party committee replacing Biden at the convention is going to create a bunch of grousing from the Bernie faction about how undemocratic the process was. And Trump will run around campaigning on saving democracy from those autocratic Democrats.

I am a psychiatrist in NYC for something called an ACT team, which is a model that exists, at least in parts of CA. This is pretty much designed for people like this (chronic decompensation/hospitalization due to treatment non-adherence), where we bring treatment to them. Somewhat insistently.
