Context? I’ve seen it too, but more when there’s an attempt to suggest action, for example “we costed it out, and it doesn’t make sense.”

I had a boss who wouldn’t let us use “and/or.” She said “or” alone includes “and.” Lawyers who once were English teachers are such sticklers!
