Just seen this. Thanks for the tag. In addition to courses Nicola mentioned (we both did the Society of Indexers one), some good extra reading is Indexing Books by Nancy Mulvany, and Chapter 16 ‘Indexes’ in The Chicago Manual of Style, which I expect you already have. Happy to help more if I can.
