I think the only woman who's broken through there at least in the console space is Jade Raymond who has her own studio that got bought out by Sony. Amy Hennig is close but her stuff keeps getting stuck in development hell.

I will say "auteurs" in gaming are rare, it's a *very* corporatized art form in terms of the products being recognized as belonging to the company (studio or publisher). Active male auteurs include Sakurai, Kojima, Sakaguchi, Schaeffer, and Levine. Few others, and "active" needs quotes for some.
