And yes, it has no pictures. And it’s wonderful to see kids have the revelation that you do not need pictures to get immense satisfaction from a book, but that words can paint the most amazing pictures all by themselves. Proving again it’s not true that 1 word = 1 millipicture.

One trial I was involved in of that design failed to reach its primary endpoint (based on numbers with stable disease) because a lot of people had a good response. This is a great problem to have: participants had great outcomes. Design then switched to a straightforward phase 2.

One challenge is you might not be sure how many you need to enrol at the start in order to get the numbers to be randomised, since that’s a subset of the whole. Makes it harder to estimate overall trial resources.

For example:
Lead-in: everyone gets treated for N weeks, then restage.
* Disease progression: stop.
* Response: continue.
* Stable disease (ie could just be an indolent cancer but might be being controlled by drug): randomise between continuing or alternative (eg stopping).
