This graph is premiums minus cost of claims. Admin expenses, taxes, etc. are not included. If they were this would not be labeled gross margins, it would be labeled net margins. Gross margins are revenue minus COGS, which is not at all the same as net.

Most definitely do struggle to break even. Gross margins are not the same as net profits. Seniors/acute care recipients have extremely high administrative costs relative to the rest of the population. This graph, seemingly intentionally, lumps those costs in as though it's profit.

These articles are misguided for clicks. The model is risk-based, so if you have certain conditions your cost for care is higher. If you have a history of alcoholism it creates a risk and a likely higher cost of care. There's no medical treatment for a history of alcoholism.

So it definitely is not. Median is the center value across an array of values sorted smallest to largest. Average is the sum of all values divided by the number of values. Saying median is a form of average is saying they're the same thing because you learned them in math class.
