I am making a note about the UK usage here, but fwiw we do use the "Prime Minister Lastname" format in Canada at least.

I think this is genuinely an American vs British dialect thing, because you absolutely would say head accountant Smith, or whatever. I mean, we don't say, Joe Biden, the President, we say President Joe Biden, Senator Warren, Secretary Buttigieg, etc etc etc.

There are some, like a school principal is called “Principal Smith”, an athletic coach is called “Coach Jones”, etc. But it’s wildly inconsistent. I think it’s like wanting non-Catholics to call the Pope “Your Holiness”.

For government officials? In the U.S., the Job Title-Lastname formulation is the default. We definitely say President Biden, Speaker Johnson, Chief Justice Roberts, Attorney General Garland, Secretary Blinken, etc etc.

Honestly I wish we’d do this with the POTUS. I always find it jarring when folks use President as a title, esp after they have left office. It’s not President Trump or President Obama. At best, it’s Former President, Donald Trump etc. For him specifically there are other titles but I shall refrain
