Just read a post suggesting that Troi's empathy is just an ability to hear the episode's background music when someone is speaking. This is now my personal canon.

In the Wizard of Oz, the characters can clearly hear the music at times, meaning it's diagetic. Of course the Witch is evil, she's not gonna be able to work in a bank with that theme song blaring the whole time.

Gene, et al thought Troi up, but TNG’s Bible must have given crap details on how to write her. It was obvious to me that Troi very early on gave redundant verbal descriptions of what we could easily interpret from the screen language, eg. Encounter at Farpoint. Later she was the ship’s shrink.
